Please provide the permit number issued to you as well as a phone number and the first and last name listed on your permit. Your permit authorizes only 3 takes so you will need to list each bird taken. For example, if you only take 2 black vultures you will only need to fill out the information for Black Vulture 1 and Black Vulture 2.
All takes must be reported no later than December 31st.
